Apply for Housing

The application process involves completing a Housing Registration Form and a Medical Assessment Form, which the application team will assess. If an application progresses to Stage 2, then our support team will arrange for an assessment to be completed either face to face or online to ascertain the applicant’s support needs and level of risk in more detail.
